1. Print-out of:

  1. Haas Scholars Program 2008/2009 online contract screen -- signed by you
  2. applicant information page, and
  3. project & sponsor information page.

First complete and submit the online application, the print out the three screens from the link sent to you in the confirmation e-mail.

2. 150-word statement on the value of an interdisciplinary scholarly community (see prompt next page). This is separate from the proposal.

3. Project Proposal (outlined on next page). Sections A-D should be no more than five pages long (double spaced, 12-pt. font), with your name and the page number in the header of each page.

4. Proof of financial aid eligibility
Submit a print-out of your Bear Facts "financial aid status" screen showing total need-based amount, with your name (hand) written on it, for 2007/2008. You must also apply for financial aid for 2008/2009. You must also fill out the FAFSA on line by March 2, 2008. First-time financial aid applicants must apply for a PIN, which can take several day to receive: see this Financial Aid Office web page for instructions and details.

If you did not receive need-based federal financial aid in 2007/2008 but have reason to believe that changed circumstances will make you eligible for aid in 2008/2009, you must submit your 2008/2009 financial aid application on line as early as possible before the deadline, and turn in a printout as part of your Haas Scholars application. You must be determined eligible by the start of the Haas Scholars Program 2008/2009 selection process to be considered.

5. Transcripts for all colleges and universities attended after you graduated from high school. This includes U.C. Berkeley, community or other colleges attended before U.C. Berkeley, and courses taken at other institutions while you were a Berkeley student. Make sure you select the option that allows your name to show on your BearFacts transcript.
Although applications will be accepted with unofficial transcripts, official transcripts may be requested before awards can be made.

6. Sponsor form, filled out and signed (download PDF by clicking here)

7. Signed letter of recommendation from your sponsor, sealed in an envelope and signed across the seal.

8. At your option, you may also include additional letters of recommendation.
