(The directions below were for summer 2008. Check back in Fall for updated application materials for summer 2009.)

APPLICATION SUBMISSION CHECKLIST

To apply for SURF, please submit the following (in order, unstapled and single-sided, please):

PART A: on-line
___ (A.1) a printout of your on-line application. The on-line section includes your (1) basic applicant information, (2) human subjects status, (3) eligibility statement, and (4) research fellow agreement. Remember to sign, initial, or enter information by hand where indicated on the printout.

Part B: off line docs
__ (PART B.1) a 3-5 page research proposal including your statement of the research question, the background and justification for your research, and your plan of action including a detailed discussion of the research methods you will employ. This is the most important part of your application.

__ (B.2) a one page detailed timeline. The summer is short. You will use your time most effectively if you identify and block out the tasks that need to be done. State your major tasks and the date by which they will be accomplished. Try to be realistic. (Your eight SURF weeks do not have to be contiguous. The SURF 2008 summer runs from May 27th through August 22nd: 13 weeks.)

__ (B.3) a copy of your most recent transcript and current enrollments (unofficial is acceptable)

__ (B.4) (if applicable) your documentation for the Committee for the Protection of Human Subjects, if you have already completed it. (In PART A.2 you will be asked to determine if your research involves "human subjects, " and to state the date by which you will complete your application for CPHS approval.) You must apply for CPHS approval by April 15th.

__ (B.5) a statement of other resources (including other grants, scholarships, faculty funds, personal funds) you will receive to support this project; also indicate those for which you have applied.

__ (B.6) Many projects require you to enlist the facilities, services or personnel of other institutions. (A study of kindergarten teachers, for example, could require prior approval of the principal.) To show your preparation, please give the names, phone numbers and e-mail addresses of the archivist, director, principal or other key people you have contacted whose cooperation will be critical to your success and the implementation of your summer research timeline. Attach letters of agreement if you have them.

__ (B.7) a letter of evaluation and a sponsorship agreement from a Berkeley faculty member who has agreed to provide oversight for the duration of your independent research project. Berkeley faculty (Professors, Associate Professors, Assistant Professors, Lecturers) who will be appointed through Fall 2008 may serve as SURF sponsors. See the "resources" page for hints on finding a research sponsor.

Be sure to give a copy of your proposal to your research sponsor, along with the faculty sponsor form. In the letter, your sponsor should discuss your project and evaluate your potential for successful completion. The letter and agreement part should come to us in a sealed envelope, signed by your professor.

__(B.8) If you wish to be considered for a need-based financial aid supplement please submit a copy of your most recent financial aid status statement and award summary, available on-line through BearFacts.
